I don't know what to do anymore...

Basically, For a long time, I've had really bad insomnia, it is so hard to fall asleep and I lie awake for as much as seven hours before falling asleep for an average of two or three hours. I can never focus on my school work or revision, and as I have two GCSEs on Wednesday - History and Physics - I'm starting to get very concerned. I had three other exams last week. Two of them I managed to sleep OK but only because I took cold and flu night nurse medication that I didn't need, and one of them I didn't sleep well before even though I took the night nurse. I planned to get the bulk of my revision for these two subjects done this weekend, but I slept so badly on both Friday night and last night that I can't focus at all, and now I'm panicking - last night I only slept between 05:00-08:00 am, despite going to bed before 11pm. I drink chamomile tea every night and don't drink caffeine after noon, but I can still never sleep! I don't know what I should do about my exam- ordinarily, I know I am fully capable of getting at least an A* and an A for them, if not two A*s, but because of these circumstances, I don't know how I'll manage to achieve this potential!
What should I do? Will alerting the school about this change anything? I saw a doctor a few weeks ago but she just advised me to try harder to sleep. Has anyone got any advise about how I should handle these exams?
